Predictive Value of Fragmented QRS in Response to Levosimendan Therapy in Patients with Heart Failure

چکیده
INTRODUCTION Positive inotropic drugs are necessary in patients with acute decompensated heart failure and systolic dysfunction and having hypotension and/or peripheral hypoperfusion despite vasodilator and diuretic therapy (1). Levosimendan is dissimilar from other positive inotropic remedies in terms of providing increased contractility without enhancing the oxygen consumption of the heart (2). It increases cardiac contractility as a means of enhancing the calcium sensitivity of contractile proteins in the myocardium. Furthermore, it leads to the opening of adenosine triphosphate-sensitive potassium channels in vascular tissues and vasodilatation (3).

Acute effects of levosimendan and dobutamine on QRS duration in patients with heart failure.
BACKGROUND Levosimendan is a novel inotropic agent that enhances cardiac contractility without increasing cellular calcium intake, so that it is not supposed to cause intracellular calcium overload and related arrhythmias. In patients with heart failure, prolonged QRS duration is associated with increased risk of mortality and sudden cardiac death. Structural changes in the left ventricle may l...
متن کاملThe effect of levosimendan on Tp-e interval, Tp-e/QT ratio, Tp-e/QTc ratio and QRS interval in heart failure patients
Objectives. Increased Tp-e interval (from the peak-to-the-end of the electrocardiographic T wave) and Tpe/QT ratio are associated with malignant ventricular arrhythmias. We aimed to evaluate the acute effects of levosimendan on Tp-e interval, Tp-e/QT ratio, Tp-e/QTc ratio and QRS interval in patients with severe heart failure. Methods. The study included 85 patients with decompansed heart failu...
متن کاملPredictive value of the fragmented QRS complex in 6-month mortality and morbidity following acute coronary syndrome
BACKGROUND Fragmented QRS encompasses different RSR' patterns showing various morphologies of the QRS complexes with or without the Q wave on a resting 12-lead electrocardiogram. It has been shown possibly to cause adverse cardiac outcomes in patients with some heart diseases, including coronary artery disease. In view of the need for risk stratification of patients presenting with acute corona...
متن کاملRole of the Fragmented QRS Complexes on a Routine 12-lead ECG in Predicting Mortality and Sudden Cardiac Death
Several invasive and noninvasive tests for risk stratification of sudden cardiac death (SCD) have been studied. Except for left ventricular ejection fraction, very few markers of SCD can be utilized routinely in clinical practice due to the lack of a desirable predictive value. The presence of fragmented QRS complexes (fQRS) on a routine 12-lead ECG is a marker of depolarization abnormality sec...
متن کاملFragmented QRS as a Marker of Electrical Dyssynchrony to Predict Inter-Ventricular Conduction Defect by Subsequent Echocardiographic Assessment in Symptomatic Patients of Non-Ischemic Dilated Cardiomyopathy
BACKGROUND Left ventricular (LV) dyssynchrony frequently occurs in patients with heart failure (HF). QRS ≥ 120 ms is a surrogate marker of electrical dyssynchrony, which occurs in only 30% of HF patients. In contrary, in those with normal QRS (nQRS) duration, LV dyssynchrony has been reported in 20-50%. This study was carried out to investigate the role of fragmented QRS (fQRS) on the surface e...
